Home and Community Care

Program for Younger People

The Home and Community Care Program for Younger People (HACC PYP) provides support to people under the age of 65 experiencing difficulty with daily living activities, and Aboriginal people under the age of 50 living in the City of Casey.

Program for Younger People

Our team of highly skilled nurses and allied health professionals can help you optimise your health, strength, and function. We also offer essential personal care and social support services designed to enhance your independence and optimise your well-being.

There are a range of services offered based on your needs and assessed eligibility.

  • Personal care (showering, dressing)
  • Shopping and meal preparation
  • Light cleaning and laundry
  • Social support and respite
  • Home and community nursing
  • Physiotherapy and occupational therapy

Eligibility

You may be eligible if you have a disability, injury or chronic health issues including:

  • Temporary or permanent disability
  • Short term impairment due to injury
  • Mental health issues
  • Medication related condition
  • You may also be eligible if you are an unpaid carer for someone who is eligible for the program.

If you’d like to find out more about Home and Community Care, have a confidential chat with one of our team members today.

  • You don’t need a doctor’s referral to receive support through the Home and Community Care program. 
  • Once assessed by our team, we will work with you to tailor a plan to suit your needs. 

Pricing and contributions

The program is funded and managed by the Victorian Department of Health who cover most, but not all, of the cost of services. You pay the remaining amount as a contribution towards the cost of your care. For full details see HACC PYP pricing schedule [PDF].

  • This pricing schedule should be read together with your Agreement with us.
  • All rates are displayed per visit. Rates are exclusive of GST. NA
  • Rates are inclusive of employee wages, supplier charges and indirect costs.
  • Our standard hours are 6am-6pm (Mon-Fri).
  • Telehealth and virtual services may be offered where clinically appropriate. 
  • You may be eligible for hardship assistance to reduce your contributions. We can help you to apply for this assistance.
  • Notification of any cancellation must be received at least one business day prior to the commencement of the service otherwise full fees will be incurred.
  • We may adjust fees from time to time and will agree the price of services or consumables by mutual consent, following consultation with you.
