Switch in 4 easy steps

You can switch providers in a few easy steps. Our team is here to guide you through the entire process, ensuring care continues without interruption. We'll also provide honest, practical advice so you can make a choice that's right for you.

Step 1 – Talk to our friendly team  

Call 1300 22 11 22 to talk to our friendly team, we’ll chat with you about your current home care needs and what’s important to you.

Alternatively, please complete the contact us form below with your enquiry.

Step 2 – Inform My Aged Care  

Contact My Aged Care on 1800 200 422 to let them know you wish to transfer providers. They will reassign your Support at Home package to Bolton Clarke.

Step 3 – Notify your current provider

You'll also need to let your current provider know that you’ve decided to transfer to Bolton Clarke. They may require at least two weeks notice depending on your agreement.

Note: your unspent funds with your current provider will be transferred to Bolton Clarke.

Step 4 – Starting your new plan

Once your Support at Home package is reassigned, we’ll work with you (and your support person if you wish) to create your new personalised care and support plan.
